CCAN is organized as a national collaborative with state-level chapters—State Level CCANs such as the Florida College of Clinically Aligned Nephrologists FCCAN—that bring together local nephrologists and stakeholders. Each state network adapts CCAN’s clinical standards and home dialysis initiatives to meet local needs while staying aligned with national goals.

Each State Level CCAN includes a governing board and a clinical governing board whose focus it to provide leadership, quality improvement support, and partnerships with dialysis programs and local hospitals and health systems.
