Our Mission
Our Mission

CCAN’s mission is to improve outcomes and quality of life for dialysis patients through clinically integrated nephrology networks and the expansion of safe, patient-centered home dialysis across the United States, and encourage transplant referrals wherever clinically appropriate.

Our Vision
